Hospital Indemnity Insurance – NEW

With the rising costs of health care, one trip to the hospital could cause a serious financial setback due to out-of-pocket costs that are not covered by a typical medical plan. Unum’s Group Hospital Indemnity Insurance helps provide financial protection by paying lump-sum benefits whenever a covered individual member is admitted to the hospital. Designed to complement most medical plans, this employee-paid coverage offers a rich choice of employer-selected options, including a Wellness Benefit to encourage better overall health management. Specific benefit amounts to complement your medical plan:

  • $1,000 for each covered hospital admission (once per year)
  • $100 for each day of your covered hospital stay, up to 15 days (once per year)
  • $150 for emergency room treatment for an accident (once per year)
  • $100 for ambulance for $500 for air ambulance transportation (once per year)

*Sample rates are based on 40 year old male non-tobacco user (approximately $40,000 income).

Disability Insurance

If you can’t work, would your bank account be disabled? These benefits can help you cover your expenses and protect your finances at a time when you’re paying extra medical bills. The Social Security Administration estimates that just over 1 in 4 of today’s 20-year-olds will become disabled before reaching age 67. Most disabilities are not work-related, and therefore not covered by workers’ compensation. STD: 14 day elimination period, up to 24 weeks of benefit. LTD 180 day elimination period, up to age 65, 3/12 pre-existing condition clause. *Cost per pay period: STD $16.35, LTD $15.52.  If you are vested under the state plan, Unum’s Long Term Disability benefits will not be payable.

Group Critical Illness Insurance with Wellness Benefit

Could your bank account survive a serious illness? Get protected with group Critical Illness Insurance from Unum. How can Critical Illness Insurance help? Critical Illness Insurance can pay a lump-sum benefit at the diagnosis of a covered illness. You choose the level of coverage — from $5,000 to $50,000 — and you can use the money any way you see fit. Cancer coverage must be selected separately when you enroll. This plan also includes a Wellness Benefit of $75 per calendar year per insured individual, if a covered health screening test is performed (including: blood tests, chest X-rays, stress tests, mammograms, colonoscopies full list) *Cost per pay period $13.73 ($15,000 benefit)
