| Basic Services: fillings, simple extractions, periodontic & endodontics, etc. | 10% (insurance pays 90%) after deductible |
| Major Services: crowns, bridges, implants, etc. | 40% (insurance pays 60%) after deductible |
| Coverage for Orthodontia Services: dependents up to age 19 | 50% after $50 lifetime deductible |
| Annual Deductible | $50 individual; $150 family |
Maximum Benefit – $1,000 per person total per Benefit Year. $1,000 per person total per lifetime on orthodontic services. TX residents have 80% basic and 50% major coverage. See brochure below for additional information.
